Rethinking Decades of Bad Science on Saturated Fats and Health

New research reported in many media sources yesterday refutes the connection between saturated fat and heart disease that has been taken as absolute truth for decades now. The article below is from Science 2.0. The researchers also found that consuming polyunsaturated fats like omega-3s also has little or no impact on cardiovascular disease.

That said, I will continue to enjoy grass-fed beef, eggs, and chicken sausages, as well as almond, sunflower, and cashew butters.


Do Saturated Fats Really Cause Heart Disease?

An international collaboration recently analyzed existing cohort studies and randomized trials on coronary risk and fatty acid intake and drew a conclusion that will surprise you if you only get your science and health news from mainstream newspapers or television - the evidence to support restricting the consumption of saturated fats in order to prevent heart disease isn't there.

And consumption of polyunsaturated fats is probably not a bad idea but there is insufficient evidence for guidelines which advocate the high consumption of polyunsaturated fats (such as omega 3 and omega 6) to reduce the risk of coronary disease.

When specific fatty acid subtypes (such as different types of omega 3) were examined, the effects of the fatty acids on cardiovascular risk varied even within the same broad 'family' – questioning the existing dietary guidelines that focus principally on the total amount of fat from saturated or unsaturated rather than the food sources of the fatty acid subtypes.

Lead author Dr. Rajiv Chowdhury of the University of Cambridge, said, "These are interesting results that potentially stimulate new lines of scientific inquiry and encourage careful reappraisal of our current nutritional guidelines.

"Cardiovascular disease, in which the principal manifestation is coronary heart disease, remains the single leading cause of death and disability worldwide. In 2008, more than 17 million people died from a cardiovascular cause globally. With so many affected by this illness, it is critical to have appropriate prevention guidelines which are informed by the best available scientific evidence."

For the meta-analysis, the researchers analyzed data from 72 unique studies with over 600,000 participants from 18 nations. The investigators found that total saturated fatty acid, whether measured in the diet or in the bloodstream as a biomarker, was not associated with coronary disease risk in the observational studies. Similarly, when analyzing the studies that involved assessments of the consumption of total monounsaturated fatty acids, long-chain omega-3 and omega-6 polyunsaturated fatty acids, there were no significant associations between consumption and cardiovascular risk.

Interestingly, the investigators found that different subtypes of circulating long-chain omega-3 and omega-6 fatty acids had different associations with coronary risk, with some evidence that circulating levels of eicosapentaenoic and docosahexaenoic acids (two main types of long-chain omega-3 polyunsaturated fatty acids), and arachidonic acid (an omega-6 fat) are each associated with lower coronary risk.

Similarly, within saturated fatty acid, the researchers found weak positive associations between circulating palmitic and stearic acids (found largely in palm oil and animal fats, respectively) and cardiovascular disease, whereas circulating margaric acid (a dairy fat) significantly reduced the risk of cardiovascular disease.

Additionally, when the authors investigated the effects of omega-3 and omega-6 fatty acid supplementations on reducing coronary disease in the randomised controlled trials, they did not find any significant effects – indicating a lack of benefit from these nutrients.

Professor Jeremy Pearson, Associate Medical Director at the British Heart Foundation, which helped fund the study, said, "This analysis of existing data suggests there isn't enough evidence to say that a diet rich in polyunsaturated fats but low in saturated fats reduces the risk of cardiovascular disease. But large scale clinical studies are needed, as these researchers recommend, before making a conclusive judgment.

"Alongside taking any necessary medication, the best way to stay heart healthy is to stop smoking, stay active, and ensure our whole diet is healthy – and this means considering not only the fats in our diet but also our intake of salt, sugar and fruit and vegetables."

People Who Know Their 'Heart Age' Make Greater Improvements to Their Heart Health

 

Heart disease is an issue in my family - my father died from it, as did his father, and my maternal grandfather, as well. Maybe it makes more sense now why I am a personal trainer?

So I am 46 years old, 6 ft 1 inch tall, 195 lbs, 31 inch waist, 135 total cholesterol (70 of that is HDL), and my blood pressure is 100 over 65 most days. According to the website that can help you determine your "heart age," I am 31. I guess that is good, except that they think I am overweight at 195 lbs (despite my waist size and, which they did not ask for, my low body-fat percentage).

Summary: New research suggests that talking to patients about their heart health using the "Heart Age" concept, a simple way of estimating and expressing cardiovascular risk, promotes behavioral changes that result in a reduction in their CVD risk, leading to improved health outcomes.
Risk scores for diseases such as CVD are usually presented as the percent chance of contracting the disease within the next ten years. The Heart Age Calculator, http://www.heartage.me, uses the same well established risk factor data, but expresses an individual's risk score as their estimated Heart Age to make it more personally relevant to the individual.

Cardiovascular disease (CVD) is the world's biggest killer(3), but doctors have long struggled to explain risk factors to patients in a way that encourages them to change their behaviour thus reducing risk. Previous research has shown that Heart Age is more likely to be understood and motivate people to make positive changes than traditional % risk scores, especially those who are at higher levels of modifiable risk(4).

Now, for the first time, researchers have shown that using the Heart Age tool to raise awareness of CVD risk promotes behavioural changes that result in a decrease in CVD risk. Researchers at the University of the Balearic Islands, Spain carried out the study amongst 3,153 patients, who were randomly assigned to one of three groups before completing an annual health assessment. One group was then presented with their CVD risk expressed as a % risk, while another received the same information expressed as their estimated Heart Age. A third control group received general guidance on healthy living only. Follow up measurements were recorded a year later during the subsequent annual health assessment.

Dr Pedro Tauler, researcher belonging to the Research Group on Evidence, Lifestyles and Health from the University of the Balearic Islands, commented "We know that traditional risks scores can be confusing. We wanted to test whether using the Heart Age Calculator to talk to patients about their CVD risk would have an effect on motivating them to adopt healthier lifestyles and, in turn, reduce their risk of developing CVD."

The results showed that patients who had been told their CVD risk (both as a percentage or Heart Age) demonstrated significant decreases in their risk scores compared to the control group, with improvements being greatest in the Heart Age group. Furthermore, patients who were told their Heart Age were far more likely to take action to live healthier lifestyles, such as quitting smoking. Quitting rate for smokers was four times greater in the Heart Age group compared to those who received the traditional percentage risk scores.

The authors highlight that the significant improvement in CVD risk seen in this study in the Heart Age group was reached with no intervention other than informing participants of their Heart Age.

Dr Pedro Tauler said: "This would suggest that the mere fact of presenting the patients with information that is easy to understand has a positive effect in engaging them to take preventive action. Heart Age is a cost- and time-effective strategy to motivate patients to adopt a healthier lifestyle that results in a reduction in their CVD risk. The simplicity of the tool and the fact that it is easy to understand are core to its effectiveness."

Sexual abuse raises heart attack risk for men

This research finding is not surprising, childhood trauma raises the risks for a whole host of illnesses in adult survivors. This might be the first time I have seen a study look at the longitudinal impact of childhood sexual abuse in men.

Men who were sexually abused as children are three times more likely to have a heart attack, even after accounting for factors like age, obesity, smoking, education level, and household income.
 
U. TORONTO (CAN) — Men who were sexually abused as children are three times more likely to have a heart attack than men who weren’t, a new study shows.

The study did not find similar results for women.

For a paper published online this week in the journal Child Abuse & Neglect, researchers examined gender-specific differences in a representative sample of 5.095 men and 7,768 women aged 18 and over, drawn from the Center for Disease Control’s 2010 Behavioral Risk Factor Surveillance Survey.

A total of 57 men and 154 women reported being sexually abused by someone close to them before they turned 18 and 377 men and 285 women said that a doctor, nurse or other health professional had diagnosed them with a heart attack or myocardial infarction.

“Men who reported they were sexually abused during childhood were particularly vulnerable to having a heart attack later in life,” says lead author Esme Fuller-Thomson, professor at University of Toronto’s Factor-Inwentash Faculty of Social Work.

“We had expected that the abuse-heart attack link would be due to unhealthy behaviors in sexual abuse survivors, such as higher rates of alcohol use or smoking, or increased levels of general stress and poverty in adulthood when compared to non-abused males.

“However, we adjusted statistically for 15 potential risk factors for heart attack, including age, race, obesity, smoking, physical inactivity, diabetes mellitus, education level, and household income, and still found a three-fold risk of heart attack.”

Co-author and PhD candidate Sarah Brennenstuhl notes that, “It is unclear why sexually abused men, but not women, experienced higher odds of heart attack; however, the results suggest that the pathways linking childhood sexual abuse to physical health outcomes in later life may be gender-specific.

“For example, it is possible that females adopt different coping strategies than males as women are more likely to get the support and counseling needed to deal with their sexual abuse.”

“These findings need to be replicated in future scientific studies before we can say anything definitive about this link,” says Fuller-Thomson. “But if other researchers find a similar association, one possible explanation is that adverse child experiences become biologically embedded in the way individuals react to stress throughout their life, particularly with respect to the production of cortisol, the hormone associated with the “fight-or-flight” response. Cortisol is also implicated in the development of cardiovascular diseases.”

Men - Eat Chocolate, Prevent Strokes


Mmmmm . . . chocolate. Science has been exposing the health benefits of chocolate, especially heart health, for more than a decade, but the assumption has been that it must be real chocolate, not milk chocolate (which only has a small amount of actual cocoa) or white chocolate (which contain no real chocolate at all). However, in this study, about 90% of the chocolate consumed the subjects was milk chocolate.

This study was done in Sweden, where milk chocolate must have at least 10% chocolate liquor and 25% cocoa solids (20% in the UK/Ireland) - in the U.S. there is no requirement for either, and most domestic chocolate contains no real cocoa.

The flavonoids in dark chocolate (35% cocoa solids minimum in the UK, but most domestic gourmet dark chocolate is more than 70% cocoa) have been shown to protect against cardiovascular disease through antioxidant, anti-clotting, and anti-inflammatory properties. Other studies have shown that the flavonoids in chocolate may reduce LDL (bad) cholesterol and reduce blood pressure.

It's unclear, however (at least to me), how milk chocolate can produce these same results - perhaps the small amount of cocoa was enough to produce these results?

Chocolate: A Sweet Method for Stroke Prevention in Men?

ScienceDaily (Aug. 29, 2012) — Eating a moderate amount of chocolate each week may be associated with a lower risk of stroke in men, according to a new study published in the August 29, 2012, online issue of Neurology®, the medical journal of the American Academy of Neurology. "While other studies have looked at how chocolate may help cardiovascular health, this is the first of its kind study to find that chocolate, may be beneficial for reducing stroke in men," said study author Susanna C. Larsson, PhD, with the Karolinska Institute in Stockholm, Sweden.

For the study, 37,103 Swedish men ages 49 to 75 were given a food questionnaire that assessed how often they consumed various foods and drinks and were asked how often they had chocolate. Researchers then identified stroke cases through a hospital discharge registry. Over 10 years, there were 1,995 cases of first stroke.

Men in the study who ate the largest amount of chocolate, about one-third of a cup of chocolate chips (63 grams) per week, had a lower risk of stroke compared to those who did not consume any chocolate. Those eating the highest amount of chocolate had a 17-percent lower risk of stroke, or 12 fewer strokes per 100,000 person-years compared to those who ate no chocolate. Person-years is the total number of years that each participant was under observation.

In a larger analysis of five studies that included 4,260 stroke cases, the risk of stroke for individuals in the highest category of chocolate consumption was 19 percent lower compared to non-chocolate consumers. For every increase in chocolate consumption of 50 grams per week, or about a quarter cup of chocolate chips, the risk of stroke decreased by about 14 percent.

"The beneficial effect of chocolate consumption on stroke may be related to the flavonoids in chocolate. Flavonoids appear to be protective against cardiovascular disease through antioxidant, anti-clotting and anti-inflammatory properties. It's also possible that flavonoids in chocolate may decrease blood concentrations of bad cholesterol and reduce blood pressure," said Larsson.

"Interestingly, dark chocolate has previously been associated with heart health benefits, but about 90 percent of the chocolate intake in Sweden, including what was consumed during our study, is milk chocolate," Larsson added.

The study was supported by the Swedish Council for Working Life and Social Research, the Swedish Research Council/Committee for Infrastructure and the Karolinska Institute.

Dads Less Likely To Die Of Heart Problems, Study Finds

Interesting study - it suggests that I am screwed, since I will never have children. On the other hand, my cholesterol is under 140, my calcium score is 8 (on a scale of 0-400+), and my resting pulse is 52. I work out regularly and I eat a very healthy diet. Heart disease is not likely.

-- Fatherhood may be a kick in the old testosterone, but it may also help keep a man alive. New research suggests that dads are a little less likely to die of heart-related problems than childless men are.
The study – by the AARP, the government and several universities – is the largest ever on male fertility and mortality, involving nearly 138,000 men. Although a study like this can't prove that fatherhood and mortality are related, there are plenty of reasons to think they might be, several heart disease experts said.
Marriage, having lots of friends and even having a dog can lower the chance of heart problems and cardiac-related deaths, previous research suggests. Similarly, kids might help take care of you or give you a reason to take better care of yourself.
Also, it takes reasonably good genes to father a child. An inability to do so might mean a genetic weakness that can spell heart trouble down the road.
"There is emerging evidence that male infertility is a window into a man's later health," said Dr. Michael Eisenberg, a Stanford University urologist and fertility specialist who led the study. "Maybe it's telling us that something else is involved in their inability to have kids."
The study was published online Monday by the journal Human Reproduction.
